Appointments and Cancellations
The first step towards a beautiful smile and overall health is to schedule an appointment. Please contact our office by phone so we can reserve time for you. We make every effort to honor all time commitments and request that you extend the same courtesy to us by letting us know 24 hours in advance if you are unable to keep your appointment.

If you are a new patient to our office, please visit our patient forms page to download our new patient forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your dental health needs more quickly than completing them on your arrival. We recognize that you can have an emergency (toothache) arise and we will do our best to respond to your problem quickly. If you have an emergency, please tell our office staff when you call. We will make arrangements to fit you into our schedule and you will be seen as soon as possible, but due to scheduling a delay may be encountered.
All patients are encouraged to call with any questions they have on dental and nutritional health concerns. However, it would be unfair to our patients if the Doctor were to interrupt treatment to answer every telephone call. The Office Staff can answer many of your questions. They will also relay your message to the Doctor, who will reply in person or through the Staff, at the earliest opportunity.
After Hours Telephone Calls
If you find it necessary to contact the Doctor after normal office hours, you may leave a message on the answering machine. Please speak clearly and be sure to give your name, phone number, the date and time you are calling and your message. Your message will be picked up and answered the next time the office is open for business.
Fees and Payments
Payment is required at the time services are rendered. Payment methods include Cash, Check, Visa, Mastercard, Discover or American Express.
Insurance will often reimburse you for all or part of your dental treatment expense. Each time you receive dental treatment, we will submit an insurance claim to your insurance company, provided you have given us the proper information. We require that you pay at the time of each appointment and the insurance will reimburse you for the amount which your policy covers.

Dr. Darryl Warren is a participating provider with Arkansas Blue Cross and Blue Shield and Cigna Dental Insurance.
UMR DENTAL INSURANCE ACCEPTED
ARKids A and B ACCEPTED (MCNA Dental for Kids and Delta Dental Smiles for Kids)
You will be expected to pay your deductible and co-pay, if applicable.
